• Conduct comprehensive patient evaluations in home settings
• Implement treatment plans focused on physical rehabilitation goals
• Monitor patient progress and adjust therapies accordingly
• Document care activities and communicate updates to interdisciplinary care teams
• Educate patients and caregivers on therapeutic exercises and health management

• Valid PT license with experience in home health settings
• Strong interpersonal and communication skills
• Ability to assess patient needs and develop individualized treatment plans
• Comfortable working independently in various home environments
• Commitment to patient confidentiality and adherence to healthcare protocols
