Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize patient engagement and patient self-management efforts. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrate personal accountability for relationship-based care, organizational mission and core values. The value you'll bring to the team: Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data. Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards. Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection. Advocates for and protects the health, safety, and rights of each patient. Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching. Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology. Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity. Coordinates and implements group visits for identified high-risk patient populations. Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services. Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives. Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy. Other duties as assigned.
