Social Worker Outpatient Behavioral Health

Responsible for coordinating the overall interdisciplinary plan of care for patients from admission to discharge. This position ensures that all aspects of patient care are integrated and managed effectively. This role collaborates with various healthcare professionals to develop and implement care plans. In addition, this position monitors patient progress and adjusts care plans as necessary.

• Master's Social Work Required
• Two or more years of Work Experience Required
• Experience with behavioral health patients of all ages preferred
• LSW required
• LCSW preferred
• Bilingual English/Spanish preferred
