The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers. The value you'll bring to the team: Provides nursing care and guidance to patients from initial contact through the continuum of care. Guides patients on treatment plans, next steps in appointments, and assists with scheduling follow-up appointments. Tracks outcomes of referrals and treatments, maintaining detailed records to measure program effectiveness. Develops and implements patient education programs and tools. Responsibilities

Requirements
- Bachelor's in nursing
- 3 experience with one year in specialty
- RN - Registered Nurse - State Licensure and/or Compact State Licensure
