Responsibilities
- Conduct thorough medication therapy reviews, assessing efficacy, safety, and appropriateness.
- Collaborate closely with physicians, nurses, and other healthcare professionals to optimize patient care.
- Provide education and counseling to patients regarding their medication regimens.
- Monitor and document pharmaceutical interventions and outcomes.
- Maintain compliance with regulatory standards and facility policies.
Requirements
- Active pharmacist license in the relevant state(s).
- Proven experience as a Clinical Pharmacist in hospital or clinical settings.
- Strong knowledge of pharmaceutical therapies and drug interactions.
- Excellent communication and interpersonal skills to educate patients and collaborate with healthcare teams.
- Ability to adapt to fast-paced environments and handle multiple tasks efficiently.
