In-Home Health Clinician - Physician, Nurse Practitioner or Physician Assistant (Full Time)

• Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required for NP/PA)
• Multi-state licenses (preferred)
