About the role

Responsibilities

  • Assesses human responses and adaptation to actual or potential health problems of the patient, family and/or significant other
  • Institutes and initiates a plan for patient care based on the initial assessment; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis
  • Recognizes changing patient condition and utilizes unit resources, chain of command and Rapid Response Team (RRT) in a timely and appropriate manner
  • Assimilates and prioritizes information sources to take immediate and decisive patient-focused action
  • Supports and maintains the physiologic stability of patients
  • Responds with confidence and adapts to rapid changing patient conditions
  • Documents all plans and care provided
  • Identifies need for education of patient, family and/or significant other and provides education through individualized instruction, the use of written materials and/or other available resources
  • Assigns nursing care to licensed staff, delegates nursing care to unlicensed staff and coordinates the flow of work
  • Calculates and administers medications
  • Coordinates and collaborates with multidisciplinary team members in the planning and management of patient care
  • Responds to the unique needs of patients and families coping with unanticipated treatment, quality of life, and end-of-life decisions
  • Monitors and allocates critical care services, recognizing the fiduciary role of nurses working in a resource intensive environment
  • Participates in discharge planning; provides instruction to patient/family
  • Maintains and promotes professional standards and development by participating in inservices, committees and staff meetings
  • Serves as a role model for compassionate care and service excellence and commits to Studer Principles and “must haves”
  • Facilitates patient movement within the department and through the facility, proactively managing patient admissions, discharges and transfers
  • Manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as required
  • Provides every patient the defined standard of care (ie; core measures, chest pain protocol)
  • Utilizes resources efficiently and effectively
  • Maintains safe environment
  • Addresses patient population-specific needs
  • Participates in Performance Improvement needs

Requirements

  • Graduate of accredited school of nursing
  • Possession of current Texas State License for Registered Nurse
