Rehab Admissions Coordinator

The Rehabilitation Admissions Coordinator performs activities and duties related to pre-admission evaluation and admission of patients to the inpatient rehabilitation unit.

• Perform pre-admission evaluation and admission of patients to the inpatient rehabilitation unit
• Review medical records to gather information to complete the pre-admission screening tool
• Collaborate with MD, case management, and nursing staff to facilitate transfer in a timely manner

• Two years experience as an RN
• Associate degree in Nursing
• RN license in AZ or COMPACT state
