Rehab/Long-Term Care Nurse (RN)

Deliver comprehensive nursing care to patients in rehabilitation and long-term care settings

• Deliver comprehensive nursing care to patients in rehabilitation and long-term care settings
• Monitor patient health, administer medications, and assist with therapeutic procedures
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Educate patients and families on health management and rehabilitation processes
• Maintain accurate and timely documentation of patient progress and nursing interventions
• Adhere to facility policies and safety standards to ensure optimal patient outcomes

• Current nursing license (RN) with experience in Rehab and Long-Term Care
• Strong clinical assessment and critical thinking skills
• Compassionate approach to patient care and excellent communication abilities
• Ability to work independently as well as part of a multidisciplinary team
• Familiarity with electronic health records and regulatory compliance standards
