Health Coach Medical Assistant

AdventHealth Senior Care is looking for a Health Coach to be responsible for facilitating a seamless health and wellness experience for our members. The primary responsibilities of the Health Coach Medical Assistant center around establishing trusting, supportive, collaborative relationships with members and their families.

• Obtains and reviews old medical records to ensure comprehensive patient history.
• Identifies care gaps and orders necessary screening tests.
• Performs initial health screens and intake for new members.
• Confirms appointments and provides instructions to patients.
• Conducts pre-visit RAF reviews to prepare for patient visits.
• Welcomes patients, provides tours of the practice, and reviews welcome packets.

• Graduate of a Medical Assistant Program
• Active American Heart Association BLS
• Certified Medical Assistant Certification (CMA), Clinical Medical Assistant Certification (CMAC), Registered Medical Assistant (RMA), Nationally Registered Certified Medical Assistant (NRCMA), National Certified Medical Assistant (NCMA), Certified Clinical Medical Assistant (CCMA)
