About the role
The Clinical Pharmacist supports the mission of the department, hospital, and health-system by preventing, identifying, and resolving medication use issues to assure patient safety, improve patient satisfaction, optimize patient outcomes, and minimize cost. May work in centralized and decentralized areas. Will perform all functions of a registered pharmacist, including accurate and timely dispensing, clinical interactions with patients and healthcare team, and technician supervision. The value includes actively performing pharmacist functions, participating in standard work parameters, maintaining operational expertise, overseeing technology and systems, managing inventories, supervising technicians, completing medication order scrutiny, drug profile review, patient monitoring, verifying medication orders, communicating with medical staff, providing patient-centered care, promoting cost-effective therapy, initiating drug dosing, managing consults, facilitating safety practices, participating in emergencies, and maintaining continuous learning.
Responsibilities
- Preventing, identifying, and resolving medication use issues to assure patient safety, improve patient satisfaction, optimize patient outcomes, and minimize cost.
- Performing accurate and timely dispensing of medications.
- Engaging in clinical interactions with patients and healthcare team.
- Supervising pharmacy technicians.
- Participating in medication order scrutiny, drug profile review, and patient monitoring.
- Verifying and entering medication orders, communicating with medical staff to resolve issues.
- Providing patient-centered care, promoting cost-effective drug therapy.
- Initiating and facilitating appropriate drug dosing and managing medication-related consults.
- Facilitating medication safety practices, including identifying and reporting adverse events.
- Participating in management of medical emergencies.
- Maintaining continuous learning and pharmacy practice development.
- Actively participating in direct patient care programs.
- Collaborating with healthcare providers and pharmacy staff.
- Ensuring continuity of care through shift presentations.
Requirements
- Bachelor's of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree
- BCLS - Basic Life Support
- PALS - Pediatric Advanced Life Support
- Applicable State Pharmacy License
