Clinical Pharmacist Tele-Pharmacy PRN

The clinical pharmacist telepharmacy provides timely, accurate, and complete pharmaceutical care

• Perform daily pharmacist functions using standard techniques and procedures to process written and electronic medication orders
• Provide timely, accurate, and complete pharmaceutical care
• Monitor medication selection and dosage by order of the prescriber or approved medical staff protocols
• Communicate with site staff when indicated for clarifications, dosage adjustments, lab monitoring and drug therapy recommendations

• 3 years current acute care hospital pharmacy practice
• Bachelor's of Science in Pharmacy (B.S.), Masters or a Doctor of Pharmacy (Pharm.D.) from an accredited school of pharmacy
• Current licensed Pharmacist within the Washing state, centralized pharmacy services are provided
• Broad knowledge of pharmacy practice in an acute care hospital setting
