Infusion Pharmacist

Assess patient suitability for home care and appropriateness of prescribed therapy Dispense medications, IV admixtures, and related supplies, ensuring accuracy and compliance with USP <797> standards Perform pharmacokinetic dosing and make clinical recommendations Document patient progress, counseling, and all communication with healthcare providers Oversee and delegate tasks to pharmacy support personnel Participate in on-call rotations and support patient care conferences Ensure proper compounding, cleanroom, and anteroom practices Support pharmacy operations, including QA/QC documentation, training, and supervision of interns or pharmacy students

• Assess patient suitability for home care and appropriateness of prescribed therapy

• Perform pharmacokinetic dosing and make clinical recommendations

• Bachelor of Science in Pharmacy required; PharmD or MS in Pharmacy preferred
• Active pharmacist license in the state of employment
• Minimum 2 years of acute-care pharmacy experience or 1 year home infusion experience
• Strong knowledge of intravenous admixture services and IV therapy
• Excellent verbal and written communication skills
• Strong problem-solving, organizational, and time-management abilities
• Ability to supervise staff and work collaboratively with healthcare professionals
