About the role
As a Care Coordinator RN with CHI health, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.

Requirements
