Infusion Pharmacist

A growing home infusion pharmacy in Carmel, IN is seeking an Infusion Pharmacist for a direct-hire opportunity. The pharmacy is open to reviewing hospital pharmacists with IV experience and may provide relocation assistance.

• Assess patient suitability for home care and appropriateness of prescribed therapy
• Dispense medications, IV admixtures, and related supplies, ensuring accuracy and compliance with USP <797> standards
• Perform pharmacokinetic dosing and make clinical recommendations
• Document patient progress, counseling, and all communication with healthcare providers
• Oversee and delegate tasks to pharmacy support personnel
• Participate in on-call rotations and support patient care conferences
• Ensure proper compounding, cleanroom, and anteroom practices
• Support pharmacy operations, including QA/QC documentation, training, and supervision of interns or pharmacy students

• Bachelor of Science in Pharmacy required; PharmD or MS in Pharmacy preferred
• Active pharmacist license in the state of employment
• Minimum 2 years of acute-care pharmacy experience or 1 year home infusion experience
• Strong knowledge of intravenous admixture services and IV therapy
• Excellent verbal and written communication skills
• Strong problem-solving, organizational, and time-management abilities
• Ability to supervise staff and work collaboratively with healthcare professionals
