Social Worker MSW Hospice Inpatient Units

Responsible for delivering various social work services to terminally ill patients and families. Provides initial psychosocial evaluation, ongoing psychosocial support, direct casework services, and referral to community resources to assist with end-of-life care needs. Collaborates with the hospice interdisciplinary team to implement, evaluate, and adjust individualized plans of care. Assists with Medicaid applications, Nursing Home and ALF placements, hospice inpatient unit placements, and financial needs. Engages in community education and outreach.

• Master's degree
• 1 year experience in health care setting
• Active American Heart Association BLS
• Valid in state Drivers License with current car insurance
