About the role
Advanced Practice Provider - Nurse Practitioner – Bilingual - CareBridge
Responsibilities
- Provides primary and urgent health care via telephone and tele video modalities
- Develops and implements clinical plans of care for adult patients
- Gathers history and physical exam and diagnostics
- Maintains contact with other clinical team members
- Maintains patient medical records and medical documentation
Requirements
- MS in Nursing
- Active national NP certification
- Valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license
- Experience working with Electronic Medical Records (EMR)
- 2+ years of experience in managing complex care cases
- Bilingual or Multi-language skills
