About the role

The Psychiatric Social Worker 3 supports the mission of DOC, Health Services Division, and Mental Health Services

Responsibilities

  • Conduct release planning for incarcerated individuals
  • Identify transition needs for incarcerated individuals
  • Conduct reentry groups and forums
  • Set follow-up medical and behavioral health services
  • Attend MD/FRMT meetings
  • Input daily OMNI encounters
  • Document all contacts with patients
  • Respond to kites and other correspondence
  • Facilitate Affordable Care Act (ACA) services
  • Participate in multi-disciplinary team meetings
  • Understand the Supplemental Security Income (SSI) process
  • Confirm incarcerated individuals as SSI recipients
  • Interact with DSHS Home and Community Services
  • Participate in networking and information sharing
  • Facilitate successful entry to prison
  • Support medical and mental health staff
  • Coordinate medical surrogacy when needed

Requirements

  • Must be able to obtain and maintain status as an Agency Affiliated Counselor
  • Current and valid WA Social Work license
  • Current and valid Driver’s License
  • 3 to 4 years of post-graduate experience
  • Desired Qualifications: Current, valid Washington State Social Worker license
