About the role
The Psychiatric Social Worker 3 supports the mission of DOC, Health Services Division, and Mental Health Services
Responsibilities
- Conduct release planning for incarcerated individuals
- Identify transition needs for incarcerated individuals
- Conduct reentry groups and forums
- Set follow-up medical and behavioral health services
- Attend MD/FRMT meetings
- Input daily OMNI encounters
- Document all contacts with patients
- Respond to kites and other correspondence
- Facilitate Affordable Care Act (ACA) services
- Participate in multi-disciplinary team meetings
- Understand the Supplemental Security Income (SSI) process
- Confirm incarcerated individuals as SSI recipients
- Interact with DSHS Home and Community Services
- Participate in networking and information sharing
- Facilitate successful entry to prison
- Support medical and mental health staff
- Coordinate medical surrogacy when needed
Requirements
- Must be able to obtain and maintain status as an Agency Affiliated Counselor
- Current and valid WA Social Work license
- Current and valid Driver’s License
- 3 to 4 years of post-graduate experience
- Desired Qualifications: Current, valid Washington State Social Worker license
