Home Health Physical Therapist Care Manager

The Home Health Physical Therapist Care Manager is a professional therapist who coordinates and directs home care patient services based on individual needs. This role involves independent management of patient populations, ensuring appropriate referrals, interdisciplinary conferencing, and comprehensive documentation. Responsibilities include evaluating patient appropriateness for home health, developing care plans with physicians, educating patients and families, providing follow-up assessments, and identifying performance improvement initiatives to achieve patient goals and top decile outcomes.

• Coordinates and directs home care patient services based on individual needs
• Manages caseload with advanced assessment and decision-making skills
• Ensures appropriate referrals to other services
• Participates in interdisciplinary conferencing
• Completes thorough documentation
• Evaluates appropriateness of home health and develops care plans with physician
• Educates patients, families, caregivers, and community providers
• Provides follow-up to evaluate care plan effectiveness
• Identifies performance improvement and standard of care initiatives

• Must be a licensed Physical Therapist
• Bachelor's Degree required
• Active American Heart Association BLS required
• Valid driver's license with current car insurance required
• Master's or Doctorate degree preferred
• Cert for Oasis Specialist-Clinical (COSC) preferred
