• Conduct comprehensive physical therapy evaluations in patients’ homes
• Develop and implement individualized treatment plans focusing on rehabilitation and functional improvement
• Educate patients and caregivers on therapeutic exercises and injury prevention
• Document patient progress and communicate with interdisciplinary team members
• Maintain compliance with regulatory and safety standards

• Licensed Physical Therapist with experience in home health care
• Strong assessment and treatment planning skills tailored to individual patient needs
• Excellent communication abilities to coordinate with patients, families, and healthcare teams
