The Home and Community Living Administration (HCLA) is hiring a dedicated Social Service Specialist to join our team in Spokane, WA.
This specialized role supports individuals within the Behavioral Support Team at Home and Community Services (HCS working with behaviorally challenging Medicaid clients residing in residential settings, such as Adult Family Homes (AFH) and Assisted Living (AL) Facilities. The population are high-risk, frail, vulnerable persons age 18 years and older who are medically and financially dependent for services from Home and Community Services.
Some of what you'll do:
Conduct functional Reporting & Evaluation (CARE) computerized tool to determine level of care and eligibility for the Residential Support Waiver (RSW), Community Options Program Entry System (COPES), Medicaid Personal Care (MPC), Community First Choice (CFC), and other programs administered by Home & Community Services for initial or on-going placement in residential facilities such as Assisted Living (AL) Facilities and Adult Family Homes (AFH)
Develop appropriate individualized service plans in conjunction with client and/or representative to meet the needs of the client
Authorize appropriate services for Medicaid clients utilizing qualified providers if the client chooses to receive their long-term care services in a residential setting or an in-home setting
Provide skilled on-going case management services to resolve issues, concerns, and diversional activities to prevent hospitalization or nursing facility placements
Coordinate and provide financial support services including assisting clients to fill out forms and applications for HCS benefits, Non-Grant Medical Assistance (NGMA), verifying Medicaid financial eligibility for RSW, COPES, MPC, CFC, or appropriateness of Fast Track services
Complete client’s estimated responsibility when appropriate
Prioritize caseload according to office policy, timeframes and workload expectations
Organize, handle and submit documents per program procedures and office specifications: ensure documents and forms are complete, accurate, and up-to-date
Participate in unit meetings, staffing with collateral contracts and community partners, mandatory on-line or in-person training, workshops, webinars, teleconferences, video conferences, and meetings
Other duties as assigned
