Registered Nurse RN Care Manager Home Health

Responsible for coordinating and directing home care for patients based on individual needs. This position independently manages the home health patient population, utilizing advanced assessment, teaching, and decision-making skills. This role ensures appropriate referrals to other services, regular interdisciplinary conferencing, and completion of necessary documentation. Other responsibilities include educating community members and patients, evaluating the effectiveness of the home care plan, and monitoring patient and family responses to achieve goals and outcomes.

• Ensures appropriate referrals to other services, regular interdisciplinary conferencing, and completion of necessary documentation.
• Educates community members and patients, evaluates the effectiveness of the home care plan, and monitors patient and family responses to achieve goals and outcomes.

• 1 year relevant clinical RN experience
• Active American Heart Association BLS
• Registered Nurse
• State Licensure and/or Compact State Licensure
• Valid in state Drivers License with current car insurance
