This position independently manages the home health patient population, utilizing advanced assessment, teaching, and decision-making skills. This role ensures appropriate referrals to other services, regular interdisciplinary conferencing, and completion of necessary documentation. Other responsibilities include educating community members and patients, evaluating the effectiveness of the home care plan, and monitoring patient and family responses to achieve goals and outcomes. The value you'll bring to the team: Provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home patients. Optimizes daily schedules to support productivity, efficiency, and best practice visit utilization. Assesses physical, functional, psychosocial, cultural, cognitive status, and discharge planning needs of home care patients. Evaluates the home environment for safety, infection control, and community resource needs. Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician. Formulates patient-specific care plans with the patient, family, and physician, establishing individualized, realistic, measurable goals. The expertise and experience you'll need to succeed: Minimum qualifications: 1 year relevant clinical RN experience Active American Heart Association BLS Registered Nurse State Licensure and/or Compact State Licensure Valid in state Drivers License with current car insurance Preferred qualifications: Bachelor's in nursing Recent, relevant experience in a Medicare-certified home health agency as a case-manager Home Health Case-Manager Certification Certificate for OASIS Specialist - Clinical This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances. Responsibilities

Requirements
- Active American Heart Association BLS
- Registered Nurse
- State Licensure and/or Compact State Licensure
- Valid in state Drivers License with current car insurance
