About the role
The Patient Care Navigator provides personalized support to patients throughout their treatment journey, ensuring they have the resources needed to successfully start and stay on their medication regimen.
Responsibilities
- Assists in setting up new patients on specialty drug therapies
- Monitors all aspects of specialty pharmacy services
- Communicates with providers regarding patient care needs
- Maintains accurate documentation of all patient interactions
- Provides assistance to healthcare professionals
- Educates patients on proper use of medications
- Addresses adherence issues
- Schedules follow-up appointments for lab work or other services
- Addresses any adverse events, medication side effects, or medication interactions
Requirements
- Bilingual in English/French
- Registered Nurse (RN) or Pharmacist (RPh) in good standing
- Superior customer service skills
- Self-directed and self-motivated
- Strong decision making and problem-solving skills
- Strong business acumen
- Excellent clinical assessment and clinical decision-making skills
- Experience in reimbursement navigation, special authorization, appeals process, and conducting field-based reimbursement support and consultation
- Knowledge of private and public reimbursement structure, systems, and process
- Strong analytical skills
- Previous experience using Patient Support Program Software/Technology is an asset
- Ability to travel when required, some overnight travel may be required
- Ability to work in a dynamic, rapidly changing work environment
- Excellent verbal and written communication skills
- Strong organizational skills and attention to detail
- Proficiency in computer applications such as Microsoft Office Suite and web-based software systems
