Health Coach Medical Assistant

AdventHealth Senior Care is looking for a Health Coach to be responsible for facilitating a seamless health and wellness experience for our members.

• Obtains and reviews old medical records to ensure comprehensive patient history.
• Identifies care gaps and orders necessary screening tests.
• Performs initial health screens and intake for new members.
• Confirms appointments and provides instructions to patients.
• Conducts pre-visit RAF reviews to prepare for patient visits.
• Welcomes patients, provides tours of the practice, and reviews welcome packets.

• Minimum qualifications: Technical/Vocational School, graduate of a Medical Assistant Program
• Must be currently certified and maintain certification by one of the following: American Association of Medical Assistants (AAMA) for Certified Medical Assistants (CMA) or American Medical Certification Association (AMCA) for Clinical Medical Assistant Certification (CMAC) or American Medical Technologists (AMT) for Registered Medical Assistant (RMA) or National Association for Health Professionals (NAHP) for Nationally Registered Certified Medical Assistant (NRCMA) or National Center for Competency Testing (NCCT) for National Certified Medical Assistant (NCMA) or National Healthcareer Association (NHA) for Certified Clinical Medical Assistant (CCMA)
