About the role
Ensures timely communication with patients, their families, and healthcare providers through professional collaboration. Collaborates with the Community Care team to evaluate patients' medical and psychosocial conditions and assess their needs. Develops and documents patient-centered plans of care based on assessment, diagnosis, and patient profiles.
Responsibilities
- Ensures timely communication with patients, their families, and healthcare providers through professional collaboration.
- Collaborates with the Community Care team to evaluate patients' medical and psychosocial conditions and assess their needs.
- Develops and documents patient-centered plans of care based on assessment, diagnosis, and patient profiles.
- Evaluates and modifies plans of care based on patient responses and attainment of expected outcomes.
- Adheres to established standards of practice and the National Association of Social Workers' code of ethics.
- Provides leadership and support, organizing program materials and promoting care coordination.
- Assists with community outreach educational programs related to care coordination.
- Coordinates care across the continuum to ensure safe, efficient, and cost-effective resource utilization.
- Identifies patient populations needing care coordination and addresses their health-related needs, including social determinants of health.
- Maintains an active caseload of 'at risk' patients and optimizes their independence through education and community resources.
- Provides crisis intervention and community support resources for patients and families experiencing significant stress due to illness.
Requirements
- Masters in Social Work (MSW)
- Minimum two (2) years experience in hospital/medical social work
