About the role
The Home Health Occupational Therapist, under the general supervision of the Care Manager, provides occupational therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient's care based on individual patient needs. The value includes performing evaluations, administering treatments, instructing patients, and ensuring patient progress is monitored.
Responsibilities
- Provides occupational therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care
- Assesses, plans, implements, evaluates total patient care
- Coordinates patient/family teaching
- Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies
- Provides goal directed care leading to specific outcomes
- May be on call for after-hours work
- Coordinates and directs the home care patient's care based on individual patient needs
- Recommends and teaches the use of assistive devices and equipment to patients and families
- Instructs nurses, Home Health Aides, and family members to provide for continuous Occupational Therapy program
- Observes records and reports on patient progress, reaction to treatment and any changes in patient's condition
Requirements
- Minimum of six-months relevant occupational therapy
- Occupational Therapist (OT)
- Active American Heart Association BLS
- Valid in state Driver's License with current car insurance
- Recent, relevant experience in a Medicare-certified home health agency as a Occupational Therapist (preferred)
- Master's in Occupational Therapy (preferred)
