• Responsible for coordinating and directing home care for patients based on individual needs.
• This position independently manages the home health patient population, utilizing advanced assessment, teaching, and decision-making skills.
• This role ensures appropriate referrals to other services, regular interdisciplinary conferencing, and completion of necessary documentation.
• Other responsibilities include educating community members and patients, evaluating the effectiveness of the home care plan, and monitoring patient and family responses to achieve goals and outcomes.

• Minimum qualifications: 1 year relevant clinical RN experience
• Active American Heart Association BLS
• Registered Nurse
• State Licensure and/or Compact State Licensure
• Valid in state Drivers License with current car insurance
