• Review and evaluate prescriptions for safety, accuracy, and clinical appropriateness.
• Collaborate with healthcare providers to optimize medication therapy plans.
• Provide patient counseling and medication education to ensure adherence and safety.
• Maintain accurate documentation and comply with regulatory standards.
• Monitor patient responses and adjust medication regimens as necessary.

• Valid pharmacist license and certification as a Clinical Pharmacist.
• Strong knowledge of clinical pharmacy practices and pharmaceutical care.
• Demonstrated ability to work independently and in a multidisciplinary team.
• Excellent communication and patient counseling skills.
• Experience in hospital or clinical pharmacy settings is preferred.
