Social Worker MSW Hospice

Responsible for performing home visits as ordered by the physician. This position prepares social histories and assesses the patient's socioeconomic and emotional status, family relationships, and coping abilities in relation to their health status and wellness potential. Additionally, this role identifies needs, prepares, implements, and evaluates individualized Plans of Care, displaying proficiency in the referral process with appropriate follow-up. In addition, this position provides high-quality counseling and resource coordination, promoting a team approach in the delivery of total patient care. Completes documentation according to Agency Policy and Regulatory guidelines.

• Masters degree in Social Work is required.
• A minimum of 1 years experience in the social work field is required.
• Healthcare/hospice experience is required
• A clear/active Social Work license for the state of State or license-eligible under State law is required.
