About the role
The Youth and Young Adult Transition Liaison is responsible for developing and maintaining collaborative relationships with DCYF-involved youth (providing direct service), community service providers, DCYF field operations, Juvenile Rehabilitation, and other government agencies to promote opportunities for youth to have a developmentally appropriate pathway to access supports and services needed to thrive as they transition out of state funded systems of care.
Responsibilities
- Partner with youth and young adults to navigate, engage and connect with state systems of care.
- Provide direct services to system-involved, transition-aged youth and young adults.
- Serve as the subject matter expert for transition-aged youth support services, and resources in their identified service areas.
- Develop positive relationships with young adults and connect them to the resources and services necessary to support their individual growth and success.
- Partner with youth and young adults in the foster care and JR system to understand the different roles professionals play in their lives.
- Act as a youth advocate and liaison between the youth and young adults, DCYF workers, and community service providers for awareness of service needs and issues related to the provision of support and services to transitioning youth and young adults.
- Provide consultation and education to DCYF workers to improve outcomes for young people with a focus on reinforcing resilience through protective and promotive factors.
- Provide support and training to the child welfare field operations and JR operations around coordinating, tracking, and facilitating transition meetings to meet the unique needs of the geographic locations they serve.
- Collaborate and consult with caseworkers, supervisors, transition planning meeting program consultants, and future planning team meeting staff to troubleshoot complex cases, analyze process effectiveness, and recommend solutions.
- Actively identify and participate in coalitions, workgroups, and case consulting teams geared towards addressing and reducing homelessness for youth exiting state funded systems of care.
- Collaborate with the Adolescent Transition Program Manager to develop, coordinate and sustain a statewide system of tracking transition programs, services and outcomes.
- Partner with the Adolescent Transition Program manager to assess and collaborate with DCYF staff, other state agencies and community organizations to develop a service array to meet unique needs of youth and young adults transitioning out of state systems.
Requirements
- Equivalent education and experience totaling six (6) years in providing direct service to youth and young adults in social service setting, case management, youth empowerment, youth advocacy, planning, policy analysis, coordination and consultation with a variety of public and private agencies, organizations, and/or levels of government.
- Associate's degree AND four (4) years of experience including providing direct service to youth and young adults in social service setting, case management, youth empowerment, youth advocacy, planning, policy analysis, coordination and consultation with a variety of public and private agencies, organizations, and/or levels of government.
- Bachelor's degree AND two (2) years of experience including providing direct service to youth and young adults in a social service setting, case management, youth empowerment, youth advocacy, planning, policy analysis, coordination and consultation with a variety of public and private agencies, organizations, and/or levels of government.
