About the role

Responsible for facilitating and decision-making for all patient transfers, admissions, and inter-hospital patient movement. This position ensures patient transfers from regional hospitals are coordinated by conferencing with appropriate doctors and completing documentation. This role assigns newly admitted or transferred patients to a bed in the appropriate patient care location in a timely and customer-focused manner. In addition, this position bases placement on diagnosis, severity of illness, and intensity of services needed. Additionally, this role maintains accurate records and documentation of patient transfers and admissions. Responsibilities for this role include coordinating patient transfers, ensuring timely bed assignments, and maintaining accurate documentation.

The value you'll bring to the team: Facilitates all admission processes related to the Transfer Center, Emergency Department, and Surgical Services. Communicates with physicians and outlying facilities to address patient placement issues and escalates unresolved concerns using the established chain of command. Utilizes nursing education, skills, and knowledge to gather appropriate patient clinical data for proper placement. Accepts and tracks all hospital transfers and transports. Assists in data collection, monitoring, evaluation, and analysis of triage problems to develop and implement protocols, policies, and procedures. Assigns beds for all requested in-hospital transfers based on acuity and patient care needs. Monitors logistics board to ensure timely movement of patients and facilitates timely turnaround times for patient rooms. Oversees and supports the work of the Staffing Coordinator and Patient Registrar, including completing annual evaluations. Assures efficient patient placement for all direct and ED admissions, transfers, post-operative patients, and intrahospital transfers, following protocols to secure timely and accurate information.

The expertise and experiences you'll need to succeed: Minimum qualifications: Bachelor of Science degree in Nursing Current State of Florida or Multi-state licensure as a Registered Nurse Four (4) or more years of Nursing experience, including a minimum of one year experience as a Supervisor, Charge Nurse or comparable leadership experience Current Basic Life Support (BLS) certification Preferred qualifications: Previous Case Management experience Previous experience in Critical Care / Emergency Department

Requirements

  • Bachelor of Science degree in Nursing
  • Current State of Florida or Multi-state licensure as a Registered Nurse
