Responsibilities

  • Conduct interviews for ongoing assessment and treatment
  • Provide crisis intervention as needed
  • Monitor, track, and report on resident and family complaints
  • Complete documentation in a timely fashion
  • Perform assessments, one-on-one visits, phone calls, care conferences, discharge planning
  • Serve as facility discharge planner
  • Maintain working knowledge and written materials related to community resources
  • Conduct and participate in resident/family groups
  • Develop behavior plans and maintain behavior monitors
  • Ensure that all psych drugs are tracked and monitored
  • Prepare reports for behavior team on assigned residents
  • Review and monitor plans with team support and make recommendations
  • Provide in-service education to all staff
  • Provide oversight, direction, training, performance development plan expectations and evaluations, and approval for leave, timesheet reporting, etc.

Requirements

  • Master’s degree in social work (must be from an educational program accredited by the Council on Social Work Education)
  • Minimum of two (2) years of experience providing care for individuals in a nursing home setting
  • Advanced practice knowledge in aging, disability, mental illness, PTSD and Alzheimer’s disease
  • Licensed Social Work Associate Advanced (LSWAA) in Washington state within twelve (12) months from hire date
  • Proficient in the English language and be able to read, write and speak English
