• Licensed Physical Therapist with current credentials to practice
• Proven experience in home health physical therapy
• Strong assessment, treatment planning, and rehabilitation skills
• Knowledge of patient care coordination and documentation standards
• Excellent communication and interpersonal skills to engage with patients and families
