With an emphasis on health promotion and disease prevention, this role serves as a vital link between healthcare providers and the communities they serve. As a trusted community member with deep insight into local needs, this individual advocates for accessible healthcare and connects patients with essential resources and services. By supporting individuals before and after hospital discharge, the role empowers patients to take ownership of their health and well-being. Responsibilities include traveling between hospitals for on-site duties as well as working remotely to maintain continuity of care. In collaboration with an interdisciplinary team, this position ensures holistic patient support and contributes meaningfully to the overall care experience through precise and dedicated execution of assigned tasks. The value you'll bring to the team: Connects with patients prior to discharge to understand contributing factors and barriers, identifying social and resource needs. Educates patients and families about available community resources, programs, and social services. Conducts post-discharge phone calls to provide support and encouragement, obtain updates on recovery progress, and collaborate with resource providers for additional assistance. Demonstrates good interpersonal communication skills in challenging situations, interacting respectfully, calmly, and courteously with patients, physicians, colleagues, and others. Identifies patient needs and helps find resources to address those needs.
