• Completing comprehensive pre-transplant psychosocial evaluations on pediatric, adolescent, adult and geriatric patients.
• Determining patient's ability to manage his or her own care post-transplant/Ventricular Assistance Device (VAD) care, including transportation, support system/caregiver, financial status, housing, mental status, and functional status.
• Working directly with the pharmacist and financial coordinator to ensure the patient has the financial resources to acquire needed medications post-transplant.
• Actively participating in patient and family support and education through the monthly support group.
• Attending and participating in team meetings that are focused on patient care and treatment planning.
• Providing case management, crisis intervention, and counseling where clinically indicated to patients pre- and post-transplant.
• Actively participating in outstanding customer service and accepting responsibility in maintaining relationships that are equally respectful to all.
