About the role

Job Summary: In addition to the responsibilities listed above, this role is also responsible for consent signing for transplant evaluation; applying standardized procedures to administer treatments; monitoring patients medication, providing intravenous (IV) insertions, drawing labs, and lab management; partnering with advanced practice provider (APP) or the primary care provider to resolve any patient concerns; and monitoring of implantable devices.

  • Supports efforts to remain updated on current research, policies, and procedures
  • Facilitating contract alignment by ensuring contracts are established for Transplant services
  • Facilitates services related to coordination of care by coordinating case management
  • Contributes to improvements to operations by identifying and reporting quality of care issues

Requirements

  • Bachelors degree in Nursing
  • Minimum five (5) years of experience in case management, transplant, acute care, or a directly related field
  • Registered Nurse License (California) required at hire
  • Advanced Cardiac Life Support required at hire
  • Basic Life Support required at hire
