About the role

The Engagement Specialist plays a key role in introducing patients to Continuous Care (CC) services. This position focuses on proactive outreach, patient education, and enrollment — ensuring patients understand the value of CC and feel supported from their first interaction.

Responsibilities

  • Make outbound calls to patients identified through campaigns.
  • Receive inbound calls from patients that received a referral to CC.
  • Assess patient eligibility for financial assistance.
  • Educate patients about the purpose and benefits of Continuous Care.
  • Guide patients through the enrollment process in a clear, compassionate, and supportive manner.
  • Utilize scheduling workflows to book initial discussions with CC.
  • Coordinate with health system staff to ensure patients are scheduled at following hospital discharge.
  • Monitor reports to identify and track provider referrals to CC.
  • Conduct outreach to referred patients, ensuring timely engagement and follow-up.
  • Partner with leadership to monitor referral trends and address any barriers to patient outreach.
  • Engage 30–50 eligible patients daily who meet CC referral criteria.
  • Assist with campaigns and initiatives designed to increase CC enrollment and patient engagement.
  • Document patient interactions, eligibility status, and enrollment outcomes.
  • Provide reporting to CC leadership on outreach activity, enrollment progress, and overall engagement trends.

Requirements

  • High school diploma or equivalent; associate degree preferred.
  • Experience in healthcare, patient engagement, customer service, or related field.
  • Strong communication and interpersonal skills, with the ability to explain complex information simply and compassionately.
  • Proficiency in electronic medical record systems (Epic preferred).
  • Strong computer skills and ability to navigate between multiple platforms.
  • Ability to multitask, prioritize, and manage a high call volume effectively.
