Clinical Pharmacist Full Time Rotate

• Provides clinical pharmacy services including primary care, consultative, and drug information services to patients and healthcare providers.
• Reviews prescriptions written by authorized prescribers to determine safety and appropriateness.
• Contacts authorized prescribers to review vague or illegible prescriptions.
• Fills prescriptions by dispensing or compounding medication according to established procedures.
• Interviews patients for previous medication history and provides counseling on current drug therapy.
• Identifies patients who require teaching and provides educational material.
• Works collaboratively with other healthcare providers to ensure coordination of patient education.
• Assigns, educates, trains, and supervises support personnel in medication preparation and related duties.
• Ensures accuracy of work completed by support personnel in compliance with policies and procedures.
• Ensures maintenance of inventory of medications and pharmacy supplies and related records.
• Ensures compliance with storage regulations for medications and supplies.
• Provides formal patient and staff educational programs.
• Conducts inspections of patient care areas to ensure proper drug availability, storage, and dating.
• Provides clinical pharmacy services such as therapeutic drug monitoring, anti-coagulation service, nutritional support, and adverse drug reaction reporting.

• Bachelor of Science in Pharmacy or Doctorate of Pharmacy from an accredited college or completion of foreign equivalency examination.
• Registered Pharmacist and Controlled Substance licensures in the State of Michigan obtained within 3 months of entry into the job.
