Advanced Practice Provider - Nurse Practitioner - CareBridge - Virtual
Responsibilities
- Provides primary and urgent health care via telephone and tele video modalities
- Develops and implements clinical plans of care
- Gathers history and physical exam and diagnostics
- Identifies and closes gaps in care
- Meets the patient’s and family’s physical and psychosocial needs
Requirements
- RN license in TWO or more of the following states: MA, VA, TN, FL, IN, OH, TX, KS, AZ, IA, NM, NJ, HI
- Active national NP certification
- Experience working with Electronic Medical Records (EMR)
